You have been selected as the apparent lowest responsible Bidder for the above referenced Project. The Regents of the University of California will award a contract (the Contract) to you by sending you a fully executed copy of the Agreement, if you comply with the conditions stated below.

Enclosed are three (3) originals of the Agreement and other University forms. You must submit to The Regents of the University of California, at the address stated below and no later than ten (10) calendar days after receipt of this notice, all of the following:

  1. Three (3) originals of the Agreement (duly executed and acknowledged by you).

  2. Payment Bond (duly executed and acknowledged by you and the surety) on University's form.

  3. Performance Bond (duly executed and acknowledged by you and the surety) on University's form.

  4. Certificates of Insurance (duly executed by insurers) on University's form, along with Enrollment Application for coverage under the University Controlled Insurance Program (UCIP), if applicable.

  5. Name of, qualifications of, and references for the Superintendent proposed for the Work.

  6. Names of all Subcontractors, with their addresses, telephone number, facsimile number, trade (Work Activity) on Bidders' company stationery. Evidence, as required by University, of the reliability and responsibility of the proposed Subcontractors such as statements of experience, statements of financial condition, and references.

  7. Preliminary Contract Schedule as required under Article 3 of the General Conditions.

  8. Selection of Retention Options and Escrow Agreement for Deposit of Securities in Lieu of Retention and Deposit of Retention (Exhibits 5A and 5B). If not submitted, University will withhold retention.

  9. Cost Breakdown

  10. {If required for qualification Method C (see RD2.9): Contractor's Statement of Experience and Financial Condition (complete and duly executed by you); current as of the end of your most recently ended fiscal year.}

All of the items referred to above must be in the form and satisfy the requirements set forth in the Bidding Documents. All such items shall be submitted to:
