Emergency (serious situation):

• Call 911 for ambulance service if necessary.
• Call as soon as possible to report all serious injuries: 1 (877) 682-7778
• If the injury was caused by equipment, remove the equipment
• For additional assistance, contact Leave and Accommodation Services  leaveandaccommodations@ucop.edu

Non-Emergency:

• Upon knowledge of injury, contact Leave and Accommodation Services and complete the supervisor’s incident report form.
